Changing Lives Fundraiser

To Whom It May Concern,

My name is Caitlin Fortuna and I am a resident of Salem, MA with my architect husband,
Darguin Fortuna, and my three beautiful children. Unfortunately, in August of 2018, I lost my
mother to a rare and aggressive form of cancer, cholangiocarcinoma (bile duct cancer).
Therefore, my siblings and I have decided to host an annual fundraiser in order to raise
awareness, raise funds for a cure, and in memory of our mother, Bonnie Fleming.
The event will be held on Friday, February 15, 2019 from 6:00 – 10:00 PM at the Salem
Waterfront Hotel in Salem, MA. There will be food, an open cash bar, fun and dancing with DJ
Nick, and a silent auction. Event proceeds after expenses will go the Cholangiocarcinoma
Foundation (a 501(c)(3) public charity, with a Platinum Guidestar rating from which over
$2.4 million has been raised from donations since the organization’s founding).

How can you help?
We are currently looking for event sponsors, event attendees, monetary donations, and
donated items for the silent auction.
 Event Sponsor – A sponsor can either donate or pay for (in full or partial) the event
venue or food.
 Event Attendees – Tickets are $23 for adults, $20 for children, children under 3 years
old are free.
 Monetary Donations – All donations made out to the Cholangiocarcinoma Foundation
on behalf of the Changing Lives Fundraiser are eligible to receive tax donation receipts
and thank you letters.
 Silent Auction – We appreciate any and all items you would like to donate! Examples
are gift cards, gift baskets, handmade items etc. All donated items must be delivered to
event host one week before event date.
