    Dear Dancer41, I am so sorry about what your husband is going through, and you with him.

    His results sound potentially promising!

    I would ask the cholangiocarcinoma specialists and any immunotherapy specialists (maybe you can contact immunotherapy clinical trial contacts through the numbers listed on their clinicaltrials.gov page and ask whether the trial investigator doctor can weigh in) whether his findings mean he should pivot to immunotherapy NOW (and if so, which one? Is a combination trial a better bet for him – like those pairing radiation or ablation with immunotherapy, or 2 checkpoint inhibitor drugs, etc?) Chemo will always be there to go back to if he doesn’t see results from immunotherapy, but my understanding is that it’s better to look into immunotherapy if it could be a promising option asap.

    Hi Dancer41,

    To my knowledge, we have had two board members post about long-term positive experiences with Keytruda.  I believe both had genomic testing results suggesting Keytruda could be effective.  You can search posts by these individuals.

    One is Matt Reidy “mattreidy.”  In a more recent post, Matt gives an e-mail address where he can be contacted.  He has also organized a Facebook page for cholangiocarcinoma patients receiving immunotherapy, which may also be a helpful source.

    The other person with Keytruda success is Rose Wong, whose daughter Colleen “ctwong81” has been posting periodic updates.

    There have been a few others posting on the board whose cancer progressed under Keytruda, and who moved on to other treatments.

    More recently, a few clinical trials have been organized to see if immunotherapy boosts the effectiveness of traditional treatments such as chemo or radiation.  These trials may not require that the patient have any specific genomic defects as might be determined through genomic testing.

    I hope this is helpful as you look into treatment options.

    This board doesn’t seem very active but was hoping someone might be able to share some perspective.  My husband has inoperable intrahepatic cholangiocarcinoma.  He’s finishing up his fourth round of gem/cis and has his first post-chemo scan this week.  We are petrified.

    We also just got his molecular results back and it looks like Keytruda may be an option for him.  He has 20% expression of PD-L1 (intensity 2+) and a Tumor Mutation Burden of 17.7 (High – Above Upper Quartile).   Nothing else turned up.  Just curious if these results help provide any insight into how effective Keytruda might be or if anyone has a similar mutation.
