Biliary PDF download link not working

I’m having trouble with the free biliary information card. I’ll fill out all the information and click submit. Once it loads the next page with the instruction and download link I’ll click the link only to have it redirect me to the homepage with no indication of any pdf.
I have tried this on Safari, Firefox, and Chrome, and on two different computers (windows and mac). I’m not sure what the issue is but would really appreciate any help you can offer.
