The COVID 19 pandemic is the most wide-reaching and devastating public health event of our lifetimes. It has reached nearly every corner of the globe. Even those who have not contracted the disease have been affected by it.

The Cholangiocarcinoma Foundation would like to know how the COVID-19 pandemic has affected your, or your loved ones, health and access to medical care. Please take the following survey to help us better serve the cholangiocarcinoma community by gathering this important information.

The survey will take approximately 5 minutes to complete. Each patient or caregiver can submit the survey only once. Your feedback on your experiences is vitally important to our community.

Because of unexpected maintenance on SurveyMonkey on July 3rd from 11am-11pm, Eastern, we are extending our deadline until midnight on July 5th.

All participants who complete and submit the survey by JULY 5, 2020 will be entered into a raffle to win one of five $25 gift certificates to the CCF Store.
